Wellness Pass： full access 🚘throughout the festival「心感·靈動」全日通

「心感·靈動」舞蹈祭，設有10個讓公眾報名參加的節目，部分節目更包含2至3個獨立單元，活動數之不盡。為了令更多人士受惠，舞蹈祭除了增設免費項目，舞盟更提供以下套票優惠:

• 單一節目 HK$250

• 兩節組合 HK$450

• 三節組合 HK$600

• 舞蹈祭通行證 HK$850 （數量有限，先到先得。不設退款或共用）

所有活動將於5月22日在art-mate 網站接受報名。

“Momentum - Wellness through Dance” provides 10 programmes for practitioners to register. Some programmes even consist of 2-3 independent units, providing countless activities for different needs. In addition to conducting some free events for the public to enjoy this festival, the HK Dance Alliance is also providing the following concessionary packages:

• Single session $250

• 2-session combo $450

• 3-session combo $600

• Wellness Pass $850/pax (Passes are non-refundable and limited on a first-come, first-served basis; No sharing is allowed.)

Terms and Conditions

1.              提議參加者穿著輕便衣服參加工作坊。

2.              活動歡迎輪椅使用者報名。參加者必須承擔參與活動之個人風險，並根據個人能力，作出判斷是否適合上課。如需進一步了解，歡迎主動跟主辦方查詢。

3.              所有活動名額有限，先到先得。

4.              活動進行期間，除主辦方授權之人員外，任何人士不得拍照、錄音或錄影。

5.              香港舞蹈聯盟保留一切權利，拍攝或錄影參加者進行活動的情況以作日後宣傳及記錄之用。

6.              每張通行證僅供一人參加舞蹈祭之活動。閣下不應與其他人分享通行證，以防止未經授權之使用。

7.              部分活動過程或會感到療癒，但所有項目均不是一節正式的輔導。

8.              所有收費節目不設退款或更改。

9.              報名表上的個人資料均由參加者自願提供，所提供的資料會用作處理相關活動的申請、緊急聯絡及統計與研究之用途。根據個人資料（私隱）條例，參加者有權查閱及修改所提供的個人資料。參加者若對本申請表所收集的個人資料有任何問題，可致電2267 6631與香港舞蹈聯盟聯絡。

10.        香港舞蹈聯盟保留更改節目及活動日期、時間、導師、內容及教授形式的一切權利。

1.        Participants are advised to wear casual wear for workshops.

2.        Wheelchair-users are welcome. Participants must bear their own risks in participating in the activities and make judgment based on their personal abilities as to whether they are suitable for the activities. If you need further information, please feel free to contact the Presenter.

3.        All activities are limited and available on a first-come, first-served basis.

4.        Photo-taking, audio or video recording are prohibited during the activity unless authorized by the Presenter.

5.        Hong Kong Dance Alliance reserves the right to photograph or videotape the participants during the festival for future publicity and record purposes.

6.        Each Pass is for admission by one individual to the festival. Any holder of a Wellness Pass should not share the Pass with others to avoid unauthorized use of the Pass.

7.        Some activities may appear to be therapeutic but none of these is a therapy.

8.        No refunds or changes allowed for all charged programmes.

9.        The provision of personal data by means of this form is voluntary. The personal data provided will be used for booking, emergency contact, statistical and research purposes. In accordance with the Personal Data (Privacy) Ordinance, participant has the right to request access to and correction of personal data as stated in the booking form. In case of any enquiry on the personal data collected via this form, participant can contact Hong Kong Dance Alliance at 2267 6631.

10.  Hong Kong Dance Alliance reserves all the rights to change the date, time, speakers, facilitators, content and format of the festival programmes.
